A request for service relief is included in the online Annual Leave Request process. However, if you need to submit a Service Relief Request separately for any reason, please use this link:  https://anglicandioceseofbendigo.snapforms.com.au/form/service-relief-request    If you are having trouble using the online form, click link below, print the form and submit it to the Registry.
